Forms to complete your child’s enrollment at Mila’s Preschool & Childcare Center, please review and complete the required forms below before your child’s first day of attendance.
For infants, additional forms may be required based on your child’s age and individual care needs. Families of children under 12 months will receive any applicable feeding and sleeping plan forms by email.
